(Tier II) Forms and Instructions
The Delaware Hazardous Chemical Inventory (Tier II) forms and instructions are available in two formats: Microsoft Word and Adobe PDF.  Forms in Microsoft Word can be viewed, edited and printed.  Forms in Adobe PDF can only be viewed and printed.  Regardless of which format you choose, and depending upon the security settings of your browser, you may be prompted to Open or Save when clicking on a link to download.  If you select Open, the necessary application (if installed on your system) will open and display the document in your browser.  If you select Save, you would need to launch either Word or Adobe Acrobat, then open the saved document from within the application.

This format will allow you to view, edit and print the forms.  You must have Microsoft Word 6.0 or higher to use these forms.  Before downloading, we recommend that you create a directory on a local or a network drive in which to save these documents (for example, C:\DETIER2).  To download, click on one of the links below to open the selected document.  Next choose Save As from the File menu.   You can use the existing document name, or same it under a new name.  Be sure that the file is being saved in the directory you created.

Tips for editing the forms in MS Word:

  1. To move from field to field, use the Mouse or press the TAB key (do not press ENTER).  The forms are laid out in a table format, so when you press the TAB key, the cursor will move to the next column, which is not necessarily the next logical field.  For example, after entering the Corporate name and pressing TAB, the cursor will move to the Owner/Operator Name field.

  2. A brief description of the data required for the selected field will be displayed in the status bar (generally located across the bottom of the program window).  To access additional help for a field, press the F1 key (not all fields have additional help).

  3. The forms are protected to prevent changes to the formatting.  If you enter information into the form while it is protected, and then unprotect it, any information will be lost if the form is then protected again.   Because the forms are protected, you will not be able to cut and paste to make copies of the forms.  If you need to do so, take the following steps:

- Once you have created a directory and saved the forms as described previously, start MS Word and open the Annual Facility Form (AFF.doc).  When finished entering the required information, choose Save As from the File Menu and enter PAGE1 in the File Name field .  Before clicking OK, make certain the file is being saved in the directory you created.

- Open the Annual Chemical Form - Non-Confidential (ACF_NC.doc), complete the information for the first reportable substance, then save the form as PAGE2 in the directory you created. If you are reporting additional substances, change just the chemicals specific information, then save as PAGE3.  Continue this process for additional pages as needed.

- After completing all forms, print each one, sign and mail them along with the other required information as described in the reporting instructions.

This format will allow you to view and print the forms (you will not be able to edit them).  You must have Adobe Acrobat Reader installed on your computer to use this option.  When you click on any of the icons below, the selected document will be opened in the Acrobat Reader where you can print or save the document to a local or network drive. Before downloading, we recommend that you create a directory on a local or a network drive in which to save these documents (for example, C:\DETIER2).   Printed forms should be completed, signed and mailed along with the other required information as described in the reporting instructions.

Tier2 Electronic Reporting

Tier II Manager™:
Tier II reports submitted electronically for the 2006 reporting year can be submitted using Tier II Manager™.  All 2005 Tier II report information has been loaded into Tier II Manager™ for facilities that reported in Delaware. As such, facilities using the system can simply update their previous information and avoid re-entering their data. Since Tier II Manager™ is an on-line reporting system, all inforamtion reported using this system can be viewed immediately by the EPCRA Program, emergency planners and responders. An added benefit of using Tier II Manager™ is that the system will accept reporting fee payments by internet checking, debit card, and credit card. The traditional method of mailing a check remains available.
